Vision Examination and Acquired Brain Injury

A vision rehabilitation examination is not the same as a routine eye examination. Acquired brain injury patients should be examined by an optometrist or ophthalmologist experienced in the rehabilitation of visual problems associated with brain injury. You may be initially examined by a general optometrist or ophthalmologist who than may refer you to a low vision or neuro-optometric rehabilitation specialist.

Your examination will not only include the basic components of the eye examination, measurements of visual acuity, refraction, and eye health evaluation, but will also include special tests as indicated including contrast sensitivity function, visual fields testing in several forms, and tests for macular sparing. An expanded history of your functional vision problems will be taken.

Some patients will require perceptual tests including the Behavioral In-Attention Test (BIT) of visual neglect, and the Vestibular Ocular Reflex. Careful testing of eye movements will be performed. Eye movement and/or double vision problems will be assessed. Testing with visual field expanders will be performed in hemianopsia patients. The Useful Field of View Test (UFOV) may be indicated in some patients with potential to drive. Glare and dark adaptation issues will be addressed.

If there is a loss of central visual acuity, a low vision assessment will be performed to determine if low vision aids will improve functioning. These may include stronger reading corrections, magnifiers, electronic technology, adaptive software, and video magnifiers.  

After the examination, the doctor will discuss your case with you to develop a rehabilitation treatment plan. The doctor will prescribe adaptive devices, eyewear, and therapy to aid your functioning. If you are working with a therapist, the doctor can make recommendations for your therapist.
